As you may know, there is a law now in affect that controls your privacy rights. It is called the HIPAA Law. Every state has it's own set of codes that applies to that state. If you have not created a HIPAA agreement you should consult a legal advisor for more information. If you have created an agreement , but have moved to a new state since signing it, you will need to have it updated. Normally you will sign a HIPAA form when being admitted to the hospital, but if you should enter a hospital unconsious, and do not have a HIPAA agreement with you...your next of kin will not be given any information on your condition. It is important to have a pre-signed HIPAA agreement signed should this happen to you.
Within this program we have created an identification card for you to fill out and carry with you. It lists the people who have signed copies of your HIPAA agreement listing their names and phone number. If you have not created a formal signed HIPAA agreement for yourself you should discuss this issue with a legal advisor who will advise you on the laws of your state.
